Polycystic Ovary Syndrome (PCOS) is one of the most common hormonal conditions affecting women of reproductive age. It can affect menstrual cycles, ovulation, fertility, skin and hair, and long-term metabolic health. The good news is that PCOS can be effectively managed with the right evaluation and individualized treatment.

At Saachi Family Health & Fertility Clinic, Kiwale, Pune, we provide comprehensive evaluation and treatment for PCOS, menstrual irregularities and fertility concerns.

What is PCOS?

PCOS is a hormonal and metabolic condition in which the ovaries may produce higher-than-normal levels of androgen hormones and ovulation may become irregular.

Despite its name, having “polycystic ovaries” on an ultrasound is not necessary to diagnose PCOS. A woman can have PCOS even when her ovaries appear normal on ultrasound.

Common Symptoms of PCOS

Symptoms can vary significantly from one woman to another. Common symptoms include:

  • Irregular or infrequent periods

  • Heavy or prolonged menstrual bleeding

  • Difficulty conceiving

  • Increased facial or body hair

  • Acne or oily skin

  • Hair thinning or increased hair fall

  • Weight gain or difficulty losing weight

  • Darkening of the skin, particularly around the neck or underarms

Some women have only mild symptoms, while others may experience multiple problems simultaneously.

Does PCOS Cause Infertility?

PCOS is a common cause of ovulatory infertility. Irregular ovulation can make it difficult to predict the fertile period and can reduce the chances of natural conception.

However, PCOS does not mean that pregnancy is impossible.

Many women with PCOS conceive naturally or with appropriate fertility treatment. Treatment depends on age, duration of infertility, ovulation pattern, ovarian reserve, partner's semen analysis and other factors.

How is PCOS Diagnosed?

PCOS should not be diagnosed solely on the basis of an ultrasound report.

Your doctor may assess:

  • Menstrual and reproductive history

  • Symptoms of androgen excess

  • Weight, blood pressure and metabolic risk

  • Clinical examination

  • Hormonal investigations when indicated

  • Blood glucose and metabolic assessment

  • Pelvic ultrasound when appropriate

Other conditions that can cause irregular periods or similar symptoms may also need to be excluded.

Treatment of PCOS

There is no single treatment that works for every woman with PCOS. Management should be individualized according to the patient's symptoms and goals.

1. Lifestyle Management

Healthy eating, regular physical activity, adequate sleep and sustainable weight management are important components of PCOS care.

Even modest weight reduction, when appropriate, can improve metabolic health and ovulation in women who are overweight.

2. Treatment for Irregular Periods

Depending on the patient's requirements, doctors may use hormonal medications or other treatments to regulate menstrual cycles and protect the uterine lining.

3. Treatment for Acne and Excess Hair

Acne, unwanted facial hair and hair thinning can be managed using a combination of lifestyle measures, dermatological treatments and, when appropriate, hormonal therapy.

4. Fertility Treatment

For women trying to conceive, treatment focuses on restoring or inducing ovulation and addressing other fertility factors.

Depending on the individual case, options may include ovulation induction, timed intercourse, IUI or referral for advanced fertility treatment when indicated.

Can PCOS Be Cured?

PCOS is generally considered a long-term condition rather than something that is permanently “cured.” However, symptoms and associated risks can be very effectively controlled.

Periods can become regular, ovulation can improve, fertility can be successfully treated, and metabolic risk can be reduced with appropriate long-term management.

When Should You See a Gynecologist?

You should consider a gynecological evaluation if you have:

  • Periods consistently occurring more than 35 days apart

  • Very infrequent periods

  • Persistent acne or excessive facial hair

  • Unexplained weight gain

  • Difficulty conceiving

  • Recurrent abnormal bleeding

  • Concerns about PCOS or hormonal imbalance

Early evaluation can help identify the underlying problem and prevent unnecessary delays in treatment.
